- Therapeutic class: Anticholinergic / Antimuscarinic Agent; Antispasmodic.
Description: Hyoscine (Butylbromide) also known as Butylscopalamine bromide. Hyoscine is the derivative of Hyoscine (Butylbromide) Hyoscine (Butylbromide) also known as Butylscopalamine bromide. Hyoscine is the derivative of Hyoscine (Butylbromide) It is of Natural origin and belongs to Tropane Alkaloid. It belongs to Cholinergic muscarinic antagonist pharmacological group on the basis of mechanism of action and also classified in Gastrointestinal Anticholinergics/Antispasmodics pharmacological group. The Molecular Weight of Hyoscine (Butylbromide) is 440.40.
Its pKa is 8.1. Hyoscine-N-butylbromide is from the group of antispasmodic medicine administered to take care of circumstances linked with seizures of the GI tract, like tremors or cramping.
Clinical Pharmacology
Biospan syrup is composed of Hyoscine-N-ButylBromide(5mg/5ml). Hyoscine -N- ButylBromide is an anticholinergic with high affinity for muscarinic receptors located on the smooth-muscle cells of the GI( Gastro intestinal) tract. Its antispasmodic action exerts a smooth-muscle relaxing/spasmolytic effect of the GI, biliary and urinary tracts. Blockade of the muscarinic receptors in the GI tract is the basis for its use in the treatment of abdominal pain secondary to cramping. Hyoscine-N-ButylBromide(5mg/5ml)also binds to nicotinic receptors, which induces a ganglion-blocking effect.
Oral absorption of Hyoscine (Butylbromide) is found to be 9% ±1. Plasma protien binding is 10%. and metabolism is reported hepatic. Renal Excretion accounts for >2% and plasma half-life is 8 hr.
Indication
Hyoscine (Butylbromide) is primarily indicated in conditions like Acute spasm and spasm in diagnostic procedures, Aspiration prophylaxis, Bradycardia, Cycloplegia induction, Drying secretions, amnesia, Iritis, Irritable bowel syndrome, Motion sickness, Mydriasis induction, Reduction of excessive respiratory secretions (death rattle) in palliative care, Smooth muscle spasm, Symptomatic relief of GI-disorders, The pain of bowel colic in palliative care, Uveitis, and can also be given in adjunctive therapy as an alternative drug of choice in Premedication in anaesthesia.
Contraindications
Hyoscine (Butylbromide) is contraindicated in conditions like Glucoma,Prostatic hypertrophy,Paralytic ileus.
Interactions
The anticholinergic effect of tricyclic antidepressants, antihistamines, quinidine, amantadine, butyrophenones, phenothiazines and disopyramide may be intensified by Hyoscine-N-ButylBromide. Concomittant treatment with dopamine antagonists such as metoclopramide may result in diminution of the effects of both drugs on the gastrointestinal tract. The tachycardic effects of beta adrenergic agents may be enhanced by Hyoscine-N-ButylBromide (5mg/5ml).
Warnings
There is no definite evidence of hazard from using Hyoscine-N-ButylBromide (5mg/5ml) during human pregnancy. However, the usual precautions regarding the use of drugs at this time, especially during the first trimester, should be observed. No adverse effects have been reported in nursing infants. In both cases, it is recommended to consult your doctor before use.
Porphyria: Don’t use if you suffer from Porphyria. It is unidentified how this drug may have an effect on your situation. Check with your physician prior to use.
Infants and kids: This drug is harmless for administration in kids at the suggested quantity.
Elderly: vigilance is recommended in the aged patients and a decreased quantity might be required.
Driving and dangerous work: Watchfulness is recommended as it might lead to giddiness, light-headedness and/or tranquilization. Keep away from such actions until you identify how it influences you.
Adverse Reaction
Hyoscine (Butylbromide) produces potentially life-threatening effects which include Seizures, Hallucination, Transient arrhythmias. which are responsible for the discontinuation of Hyoscine (Butylbromide) therapy.
The signs and symptoms that are produced after the acute overdosage of Hyoscine (Butylbromide) include Coma, Blurred vision, Hallucinations, Dry flushed skin, Delirium, Ataxia, Restlessness, Rapid & irregular pulse, Decreased salivary secretions, Increased heart rate, Loss of accomodation, Flushed skin, scarlet, weak pulse, Excitement.
The symptomatic adverse reactions produced by Hyoscine (Butylbromide) are more or less tolerable and if they become severe, they can be treated symptomatically, these include Dizziness, Thirst, Headache, Drowsiness, Fatigue, Restlessness, Palpitation, Tachycardia, Constipation, Dry mouth, Nausea and vomiting, Blurring of vision, Mydriasis, Loss of accomodation, Transient bradycardia, Delirium, Swallowing difficulties, Urinary RetentionX, Cycloplegia, Dry mouth, dizziness.
Symptoms of Overdose and Antidote
Symptoms: Serious signs of poisoning following acute over dosage have not been observed in man. In the case of over dosage, anticholinergic effects such as urinary retention, dry mouth, reddening of the skin, tachycardia, inhibition of gastrointestinal motility and transient visual disturbances may occur and Cheynes-Stokes respiration has been reported.
Antidote: In the case of oral poisoning, gastric lavage with medicinal charcoal should be followed by magnesium sulphate (15%). Symptoms of Hyoscine-N-ButylBromide(5mg/5ml) over dosage respond to parasympathomimetics. For patients with glaucoma, pilocarpine should be given locally. Cardiovascular complications should be treated according to usual therapeutic principles. In case of respiratory paralysis, intubation and artificial respiration. Catheterisation may be required for urinary retention. In addition, appropriate supportive measures should be administered as required.
